The single-dungeon-exit thing is older than the reporter's version by several months (0.13-a0-1298-gce26f17 versus 0.13-a0-2776-g5b28ffd). My guess is that the exit was originally an escape hatch up (placed because of the disconnected zone) that was converted into a dungeon exit by the builder's stairs fixup.
Two issues:
* D:1 shouldn't have been generated as disconnect as this in the first place.
* If we do need to place a safety hatch on D:1, it should be a hatch down, not up as on other levels, since going up loses the game. Alternatively, we could just veto D:1 if we would place such a hatch.
