Can we get Y/N prompt when cast Borgnjor's revivification?

Can we get Y/N prompt when cast Borgnjor's revivification?

Borgnjor's revivification is powerful lifesaver but it has one crucial drawback: It reduces caster's maximum HP permanently, and it cannot be recovered.
Because it's has huge, permanent drawback, I think we need to prevent casting it accidentally.
Simple Y/N prompt will be enough for fail-safe, I saw similar proposition in mantis but id does not looks like applied.

While we're at it, since it would be a simple addition: can a prompt also be issued for Death's Door? Nowadays I just reassign the key, but in the past I would forget a spell and then learn it, and then forget that the key is no longer mapped to the old spell and accidentally cast it.

Even then, it's easy to make that mistake when you have 20 spells and don't want to look at the spell screen every 10 seconds and accidentally hit the wrong key. It's not fun running away because you suddenly have 15 HP.
