These days, the devs don't really like spoilery vaults where you'd have to dig in places where it's not obvious to dig. If there's nothing revealed through a window, and you can't get inside coming up from the next level, then it's probably just a big block of nothin'.

Well except one vault where you have to actually dig a wall behind a runed door to find a bunch of dangerous monsters. It does warn you with runed doors, but the first time I had it, it was certainly strange to find just layer of wall behind the sealed door.

Right: the wall behind the runed door is a fairly big clue something's up!

There's another vault that looks like yet another one of those several paths spiraling into the center... but when you get to the center, you don't see the other paths. You can dig in that one too.

For some vaults you still have to go up some stairs from the floor below, but that's usually indicated by there not being three staircases going down on that floor before you find the ones in the vault.

I also came across a vault yesterday that I've never seen before. It was in the lair, a ice dragon was hanging out on level 6 which I thought was odd. After I killed it I found a huge pile of loot in the corner where it spawned so I assumed the loot and the dragon went together. I thought that was pretty cool and appreciated the challenge of an OOD + loot. Just saying :)
