I made a bug report with the save backup included; hopefully that suffices.

Checked for other ways to get out? Unwield weapon of distortion to get banished? Learn Passwall or Lee's Rapid Deconstruction even with high fail? Translocation miscasts?

LRD doesn't break walls anymore... If you could summon (scroll of summoning / Shadow Creatures) a deep troll earth mage or some monster (e.g. boggart) who could summon one for you, you could anger it and maybe have it break walls for you. Or a Cacodemon. But I guess D:11 is too low level for such monsters...

Cacodemons and eyes of devastation don't dig with energy bolts anymore; they stopped in commit 85f13f00f2816064f2b3d4dc3b800213a65987e2 which also stopped IOOD digging rock.

The conversation moved to mantis, but we fixed this save manually before most of the followups in this thread (by editing the save to move the player to a location outside of the vault). I did check their inventory first in case there were any unexpected ways of getting out, and didn't come up with anything.
