Poison stacks

Can someone tell me the mechanism for poison stacks on monsters? How many levels are there? On each turn which a monster spends in a poisonous cloud, does the poison level increase by 1? What about poison arrow, or venom bolt? Does one hit with these spells give 1 level of poison, or does it depend on damage?

I have been recently experimenting with poisonous cloud, olgreb's toxic radiance and ignite poison. It seems to do decent damage, but I am not sure of the mechanisms involved.

I have looked at this page on the wiki, but it seems quite old.
