Patashu is correct -- there's a gap of 40 piety between 6* and max piety, and Ru will continue to offer you sacrifices within this window, because all five of Ru's powers become more effective as you gain piety, mostly linearly. It's entirely possible to make two sacrifices and not go from 160 to 200 piety if the sacrifices are trivial enough.
If you decide you've sacrificed enough and stop making sacrifices, Ru will start offering new sacrifices after longer and longer timers in order to avoid spamming you with unwanted sacrifice messages (and prevent abusing rerolling sacrifices). You can reset the timer to its normal speed by making accepting one of the offered sacrifices.
Sacrificing a hand doesn't directly impact anything about one-handed weapons -- there will be no penalties to their use. You will of course be unable to use a shield also, and you will also lose a ring slot. Losing a hand is the largest single sacrifice by a large margin -- unless you sacrifice a skill trained to very high levels.
Edit: Slings, blowguns, and Throwing are the only one-handed "launcher" skills -- sacrificing a hand will prevent all use of bows and crossbows larger than the hand crossbow.
