This isn't quite a YASD because I was just simply in over my head. But I found it infinitely amusing.

The story behind it? I got caught by a pack of yaks, and a rat came in behind and got in the one open spot. He managed to get the killing blow. Somewhere there's a rat that's five or six levels higher than it should be.

I want to see more stupid or embarrassing morgues!

This one was also pretty bad, and entirely my fault. A pretty strong melee character with 7 runes got killed by 1 berserk guardian. Pretty sure I was there just for exp too.

One of my first air elementalists got killed by the very first bat on D1. Every single shock I cast missed it or miscast. I was running around in circles as to not find any other creatures and it just wore me down and eventually killed me.

I had taken off my Cloak of Preservation to ID the ratskin cloak, then tried to butcher a eye of devastation corpse bordering 3 slime walls, and when I started melting I thought "I'll just walk through this gauntlet of slime, no need to blink!"

All this after I had killed the Royal Jelly in 2 shots via Icestorm.

And a trample/tornado wearoff/drowning combo that you have to see the action list to believe

Don't have the file, but I got killed by a dart trap once. I'd managed to get out of my depth (I thought the food clock was MUCH more unforgiving than it is), got forced to retreat down a hallway that I'd never gone down, and triggered a dart trap which did just enough damage to kill me.
