Actually probably my most pathetic was a HOEE. First monster I saw was a cockroach - and I was able to rest my MP back to full multiple times while going through MY ENTIRE STOCK of stones casting sandblast without being able to kill it. Then I went back up the stairs in shame, so not technically a death...

Waiting out poison with potions of heal wounds, but not [s][s]healing[/s] curing[/s] healing (dangit, I'm too lazy to conform every time something else gets renamed).

Forgot that red poisoning can do more than 1 damage a turn.

My most pathetic death is as a spriggan venom mage in tartarus, getting cornered in a small loop of hallway within the radius of a silent spectre. I hadn't trained any melee skill or brought anything that could have been useful to me while silenced (wands of digging, disint, tele, damage wands, evoke for tele, evoke for blink, etc etc etc). So I just mashed my face against a skeletal warrior until I was dead. 13/15 runes. Use protection, kids!

Low pre lair level 'zerker. Comes across a melee ghost and prince ribbit- something I can't handle myself by just berserking. So I do the logical thing and call a brother in arms, charge in, and 'zerk. (My piety drops lows enough I can't call another).

My brother in Arms takes off in the other direction after a butterfly.

That's right- I got killed because of a butterfly.

We have a butterfly death on CDO because Xom inner flamed a butterfly, killing the PC with an explosion (it shows as killer=butterfly).

As far as I know killer=butterfly was deliberately scummed for.

When you have to work that hard to die in an unusual way, there's nothing pathetic about it. Like mageykun said, that is closer to the side of awesome.
