But what do you think - what should really never happen in crawl? Here are some examples.

A definition is never a set of examples. I don't know what you mean by "what should never happen." Do you mean theoretically, "these events are impossible in the current version," or normatively, "it would be really awful if such a thing happened"?

Edit: I should add that in either case there are infinite examples and it's trivial to invent as many as you want. What's the point?
