YARSD - Telport madness
This one is a really, really stupid death: I enter the Vestibule with a summoner and a couple of dragons. The fight does not get on very well, and I have to run away from the stairs and take a couple of fire hits down to low HP. So I put on that ring of teleportation and evoke it to jump back to the exit portals. And know what? I _forget_ to cast Teleport Control and get dumped into a pack of hell knights. Needless to say that the character dies without ever getting another turn.
Luckily it's a Felid, and the _second_ attempt to teleport out works fine. The lost XP hurts though.
