Quatzal or whatever his name would have been a good candidate to adding elemental flavour to unarmed attacks. A conjuration/charms spell could do the same for a small amount of time, however. Maybe it would be better to work in this direction. Adding some artefacts with flavours would also be nice.

BTW, long blades cross trains with short blades, and axes cross trains with m&f. I have sometimes gone to the Lair with a sb in lack of a flaming lb, and it didn't go badly. Even for weapon users, this means that there are alternatives. The only problem I couldn't solve was with a troll fighting...

The ability of fitting in a single square could be added to all tiny monsters, as long as they are of the same kind. Or maybe not, since killer bees are terrifying enough as they are. But such an ability would be useful for weak pack monsters (bats, flying skulls, rats...)

Having stairs allow all monsters in LOS to follow the PC, appearing one by one on the upper floor after those adjacent to him, would make stairdancing less safe and give it more of a tactical value, as it would give some initial breathing room, but also allow packs to reunite on a different terrain....

How can a melee character handle them? The only thing I can think of is knowing the AI or having a way to produce clouds, but not everyone has those. Right now, it seems to me that removing see invisible would need to make permanently invisible monster rarer, so that they can be made visible by luri...

Maybe some cemetery vaults could be added, with a variable number of depth appropriate levelled ghosts. Other ghosts could be limited to Hell, Tomb, and Pandemonium, where you already need good survivability. I also like a massive number of player ghosts spawning while running with the Orb.

River rats work well in sewers, but have nothing special in the Lair, and I also find them kinda boring. Maybe they could inflict sickness?
There could also be some "wailing rat" or somesuch that can hasten the others, and call in bigger, badder foes while the PC is swamped by little rats.
