Perma lev isn't really a requirement, but if you are already slow then I wouldn't risk it without. Thats just me though. Do mutagenic corpses have a higher chance of positive mutations than mutation pots? The last few times I've eaten them, I've certainly gotten better mutations than with the pots, but it's likely just coincidence.

You can cast confuse at the vampire mosquitoes and then stab them. You pay the price for using vampiric weapons when you equip them; there's no penalty for unequipping, except that you have to pay it again if you re-equip it. That randart dagger sucks.
