I never died to Nessos. If terrain is not optimal to fight him, I usually pop in a Blink scroll and then teleport away. Let the children come to me.

I would rather increase starting Sh skill for fighters until we got a rebalanced Sh skill. Currently the only people using shields are spellcasters with bucklers. Something is very wrong with Sh now, I think.

I don't think so. The weapon is so weak (except maybe for the first levels) that you'd want to move on. The fact that it's not dependable makes it a nice Xom item, but no more I think.

I got the staff of Olgreb two games in a row back in 0.5. The evocation is useful for berzerkers. Trog gives weapons outside of your skill all the time. I don't think it's a bug.
