I think the problem with item weights/carrying capacity/etc is that it requires tedious stashing of extra ammo and the like. If you were just slowed by wearing excessively heavy armor, that might be fine. (We already have that system for ranged combat, after all.)

Where does the license say derivations must be free of charge? The code is under GPL v2, which requires derivations to be, among other things, freely licensed; but your proposed movie wouldn't really be a derivative work of the code, I wouldn't think. But IANAL.

I believe that Sandblast has been changed to no longer use stones (so, in particular, not giving them stones doesn't gimp anything).

You can still retreat before you're in melee range, which is the best strategy anyway for speed-10 melee-only enemies one isn't yet ready to fight. Well, and foresight, which would be very powerful. Though there would be issues to iron out, since for it to make sense it would have to only be guaranteed valid if nothing relevant changes, and then what counts as relevant needs to be clear.

I suspect Flash Forge would be tough to tune so as to make whether to use it an interesting decision. It might make more sense to make it cheaper but temporary.

I thought it meant that the shield penalty only went to 0 when you got to Shields 27; and, in return, it no longer applied to to-hit. What that means for equipment choices, I don't know either.

Could just make stones, arrows and bolts never mulch. Sling-bullet mulching is potentially interesting as petercordia said, and non-stone throwables are balanced around being scarce.

I'd say not; or, if so, the risk should be communicated (this is the first I learned of it). But maybe it's for symmetry? After all, enemies hit each other with friendly fire all the time.
