I believe Angband and the games built on it do something like that, and if you take off an item with a full inventory it lands on the floor? Been a long while since I played a band though.

There's an upside; when using it early while you're fragile, it has a consistent radius so you can use it on adjacent enemies without accidentally hitting yourself or your allies.

It has; Necromancers used to cast Inner Flame. It wasn't exactly good for their mortality, or that of their allies. Might be saner in places like Gehenna or Volcano where most monsters have rF anyway.

My ideal state would be if the only ghosts the game ever generated were fabricated, since then you keep the flavor of the dungeon having claimed previous victims and the mechanical consequences of undead uniques, but without the walking brick walls and metagame nonsense and potential for griefing.

Is waiting on the stairs for monsters to come to you really a good idea? It means you're giving the monsters more time to pick crap up off the floor and potentially use it against you, or to set of alarm traps and get the entire floor doing this.
