I've never tried Statue Form and short blades. Hopefully someone else will answer.

I think any of the body armour except the the scale mail are good. I like shields for more defense, too.

Protection is not bad for a Dr.

Poison breath is strong at this point of the game. Use it.

Okawaru's Heroism and Finesse are great. Heroism you should use in every non-trivial fight and Finesse in the tough fights. I like about 10 Invocations soon for a bit more reliability and duration.

Tomb is very hard, but is predictable. The rest of extended is generally easier than Tomb, but has some much nastier spikes that are less predictable. They both should be fine. There's no need to switch God's on the DD, but consider switching weapons as vamp is almost completely useless in extended.

That seems like a great run for your first time in extended.

Pan is not too different from the Hells. Tomb is different and nasty, so be careful there next time.

Silence doesn't do anything to demon casting. Dispel Undead does really hurt. You just gotta kill the casters. Big spells like Tornado help. That guy looks really strong and well setup for extended. If you want to and can tolerate a death, go for it. Gifts drop your piety, IIRC, so you're just not seeing the time at 6*. Like fingolfin said, it doesn't matter at all.
