I dont think there is anything "painful" about playing any dd, quite the contrary, it is a smooth care-free run. If anything, its just boring. Your pain is just you trying to play a highly resourceful race conservatively.

The only thing making my hunter games take longer is having to spare ammo. Still, they only take 2-4 hours which is pretty close to melee.

Well, you could just make it so that the player stops gaining skill experience after xl27, or give the player a limited number of skill points he can learn during the whole game. Extended is in the game because its optional, it shouldnt be a problem to those not willing to do it, at least it has not...

I think of it in the opposite way. DoT is fine, it's kiting which is the problem. Of course you do. The control over the monster goes from (a) > (b) > (c) - not always, but in most cases. Really (and i have said this before) all that it takes to make vm not-tedious is a low level summon that can soak hits and suplement damage while the poison damage does its work. If it is immune to poison it can do double duty up through mid lair it it has enough hps. Replace summon forest with sum...

They were designed to be dangerous together, so that when you see a yak or killer bee pack you dont go "hum, i should get in between them and see how it goes". You never want to deal with the whole pack unless the game forces you to. Troll shaman/spriggan packs are good examples of that.

I played games where i learned silence and used it mostly if not only on elven halls and tomb - before scarabs and animated weapons inhabited those places - and even back then i felt like i was just setting myself to fail because of how silence works against you as its duration expires.

I dont see the problem in map's layout alone, i think its the whole "pack" that you would have to consider problematic, the map layout, the enemy's design that makes fighting certain combinations of enemies a death sentence, the stealth and noise mechanics, the respawn of enemies.
