I think permamark would be interesting to try out, but you should probably make them very stealthy, maybe even give prompts on noisy actions, because it can easily get them killed. Besides teleportitis it can probably give berserkitis, horns/antennae that will restrict helmets, fraility, etc. I probably wouldn't want any of those even if they come with something good. There is a very high probablity of acquiring one of those, because the chance is about 1/4.

I don't think that it's minor, for me flavour is very important. Maybe we could at least have orcish and elven tiles on armour found on those, but it would be called the same and the description would be the same? You know, like cloaks, robes, etc. look differently on the character doll.
