Summary 0007424: nicolae_vaults_meeting is an autoexplore deathtrap
Description Ran into this on my last win: if a door generates as an entrance to this vault, there's a very good chance that the wardens will immediately lock it behind you. If you happen to not have a way to destroy the door, your options are pretty much 'teleport or die'.

Sorry.

The layout of this entrance was designed to make it more likely for a warden to trap you, but perhaps the warden's backup crew is too strong?

I can make a milder version if people want one.
