Would it help to delete his vulcano map file, if it's indeed bugged? And how would I do that with the new save-game format?

Note: crawl --edit-save the_save_file.cs rm level (where the level is what used to be extension)

I removed the map from the save after Kilobyte verified it would create a new map. Please try and let me know, Noom.

removal map helps. i got instantly into random point of new generated volcano vault map. tnx

That should not happen, will look into the vault definition.

Fixed. One of the caves was indeed missing an exit. Oops!
