This is still an issue: it's present in this 0.26 Beogh morgue:

I have three instances of the string "Your ally died" in my winning morgues; in two cases I know the ally was charmed/enslaved by wand, but the third game is a (vanilla 0.20) HO^Beogh. It seems likely to me that Beogh allies can also trigger this bug.

I haven't had any luck reproducing it, but I guess monsters can be killed in a way that unsets mons->mname before we get to take_note().

Here's an old related bug, in additon to the name missing in the note, the unique ally (Urug converted under Beogh) stayed visible in Ctrl+O overview after death:
