Did the shaft place you inside the wall? In the 1st screenshot (I assume that's before/after) it appears as if you are inside the wall (which should be permarock per the vault definition) and you stepped into the vault chamber. Were you able to escape?

He clearly landed on top of the leather armour when he got shafted (read the messages).

Yeah, all those messages are from one turn. I must've fell on the armour and got pushed south. I decided to fight the mimics and afterward I wasn't able to escape.

The vault in question, xom_gifts_altar, had no_tele_into set on all the items. So this should have been impossible _unless_ mimification removed those flags. In any instance shafting and teleporting should probably be prevented from landing you on a mimic'd square.
